Well, the question pools are not ARRL's HQ problem, the National 
Volunteer Examiners group writes the pools, and approves changes and 
corrections.  Yes, there have been continuing bad questions and answers. 
  They fix one, and another sneaks in.

Any ham can challenge a question for a pool at the periodic revisions, 
there is a published procedure known to the VEC groups.
