I've always taken it to mean ATNO=Never worked that DXCC entity in
any way whatsoever. A "New one" might mean that but I take it to mean
a "new one" in a mode or band. Ie: I've had Navassa for the last 30
years but on 15M CW only. It was a "New one" to me on any of the
other bands & modes. I didn't work K1N on 15 CW this time as it was
worked there & not a "New one".
Someone else's definition will be right to them.
