I'm trying to visualize an 18" long eye bolt used on a 4" post. I would
try to have the eye as close to the post as possible to minimize bending
concerns.
Maybe you are planning on drilling staggered holes so that the eye bolt
will be in line with the middle guywire(s)?

Rohn part number 260004p. The lowest price I saw was ~$100 and looks like list
is $128. And like you said, out of stock everywhere.
I'm not clear on where/how this gets used with the equalizer plate, but it
seems there must be a better option. I'm surprised nobody has offered advice on
that.

I highly recommend Atlantic Bolt for your all your tower bolt needs. They are
semi local to me and I got to take a quick tour of their facility when I picked
up the bolts for my HDX-555.
Their pricing was very reasonable and when I picked up my bolts they supplied
full mill test / ASTM certifications for the bolts.
