If you want to put a elevated guy anchor , i have the
idea that you want at least 6 ft over the ground,  i
have 3 elevated anchor ,2 at 7ft  and other 6 ft 
supporting a 134ft tower like rohn 45,  i do this ,
overkill  anchors, I beam in one 12 ft long  6 inside
concrete 6 above, I beam  12x14x12 i think 7/16 wall
reinforced at the middle with another smaller 1/2 inch
wall I beam one side , round solid Bar 1 1/2 inch
diameter in the other side just in case , i weld
rebars to give more strengt to the concrete, talking
in ready mix bags will be a big number  at least 3
yards of concrete ( i say at least , plus big rocks 
to fill the hole) a simple rebar cage . 
the others ones are 6 inch 1/4 inch wall , double 2
pipes welded together one behind the other facing the
tower filled with concrete and 2 rebars inside, just
in case i link the pipe to the building next  to it
using 1/2 inch cable  just in case, the same concrete
, same hole

this works for me and i can sleep in the high winds

if you want to put the 3 foot of the pipe in the
ground  put at least  5 feets of concrete around the
pipe (24x24x60) above the ground to have a safe
elevated anchor,  if you base is below the surface
must be a 36x36x48 at least  imagine how many ready
mix bags you need.

obviusly i'm in Mexico i don't follow your county
regulations that you must follow i think
