[TenTec] Omni V upgrade to V.9

Since Carl has the chips, again, I am going to install one in my Omni V.

I am wondering if anyone has the labels that came with the chips, back in the 
day?  How about a word doc or .pdf with the labels?

A generous ham sent me a word doc, a few years ago, with home made labels that 
looked great.  Meanwhile, Carl ran out of chips and my old computer bit the 
dust, so the labels were lost.
