I do all those things to avoid clotting problems, but it is undeniable
that the risk is still there. And at least in my case, sleep
deprivation two days in a row almost invariably leads to a much greater
risk of catching some sickness bug due to a weakened immune system. I
am perfectly healthy and I've been an athlete of one sort or other
(endurance included) my entire life, but to pretend that everyone has
the same physiology is misguided.
I admire the capability of contesters who naturally need less sleep (I'm
aware of several who typically sleep less than 5 hours per night all the
time) and can operate a full 48 hour event, but for many of us that
simply is not feasible.

There are things to prevent that from happening.
In the case of pooled blood in the legs, compression socks, pants can help.
Also standing up from the chair at regular intervals. Short walks inside
the shack.
Sleep deprivation should not be harmful if done occasionally.
I like 48 hour contesting way too much, but I'm not against reducing the
operating time either. Whatever is best for the hobby is fine with me.
If the 48 hour format is changed to a 36-hour contest for example all I
would like to see is that those who want to keep operating are allowed to
do so (but only the best 36 hours will be computed) for example..

There is nothing at all healthy about pushing yourself like that.
Seriously.

48-hour contesting is indeed healthy.
It forces you to train your body as hell if you plan to succeed.
You gotta eat right. You cannot drink alcohol (some times hard to achieve,
but well...)
Being comfortable is not healthy.
