The SS exchange emulates the preamble/header of an ARRL format message
(traffic) so each element is required. Sending the call a second time isn't
all bad because it helps the receiving station verify what they think is the
right call as what's being sent.
It's utter madness when DX stations (W's too) run for periods and don't sign
their call. A bad spot results in a massive pileup by guys thinking they
have a new one and the running station gets to fill up his log with dupes
until he realizes what's happened.
Now for the example with w1xyz, he should follow the same concept of signing
his call after each QSO. The exception (and I do this at times) is when you
hear a weak guy calling who keeps getting beaten by louder guys. If I don't
sign my call at the end of a QSO then the pile is a little thinner and that
weak guy can be heard. Of course after that Q, I sign my call.

You are right in my book too. But one exception is sweepstakes. I don't
know the rationale, but some years ago the ARRL contest manager ruled that
in sweepstakes only discrete complete exchanges are allowed. This extra
requirement doesn't seem to appear explicitly in the current rules (at least
that I can find), but I remember a lengthy thread on the subject on this
reflector.

That depends. If the receiving station heard the callsign, then the callsign
was sent, even if people who came late to the frequency didn't hear it.
