Most of the recommendations so far have been footswitches with the pivot
away from the line cord. I'm probably in the minority in that I prefer the
pivot to be near the line cord. In fact, this is my favorite footswitch,
sadly no longer in production:

I actually like the fact that it's lightweight, so I can move it easily
to different spots under the desk to vary things up during a weekend and
reduce overall fatigue.
