One situation where it is wise to always send both calls is when the timing,
etc. leaves any doubt whether he came back to you.  For example:

CQ TEST K9MA
DL1ABC  (way off frequency)
DL1ABC K9MA 599 04
K9MA 599 14
TU K9MA

or

CQ TEST DL1ABC
K9*%$# 599 14 (qrm)
DL1ABC K9MA 599 04
K9MA TU DL1ABC

In both these cases, the extra call with the exchange indicates that the
operator is unsure about something, and the other confirms.  If the other
operator has his act together,
this is faster that asking for a repeat.  In the first case, a simple "?"
would get
DL1ABC to call again, which pretty much confirms he's calling me, but the
timing could still be a coincidence.  In the second case, "?" is likely to
just get a repeat of the exchange, still leaving some doubt whether he
actually called me.  

With CQ now throroughly cross-checking logs, it's going to be worthwhile to
take a bit
of extra time to cut down the number of broken QSO's!
