Reading from the schematic in the manual:
1 - Motor winding end
2 - Position potentiometer wiper
3 - Motor winding center tap
4 - Positioning potentiometer
5 - Positioning potentiometer
6 - Motor winding end
The pot is shown as 75 ohms.

It is a 6-wire screw terminal on both the controller and the rotor.
