The basic information is in Rothammel's Antennenbuch, with formulas for
both 300 ohm and 75-ohm phasing lines. The book has been put into a
second edition and is available from a German publisher who advertises in
most German ham journals. I hope someday for US and other
English-speaking hams that it is translated--it is a classic compendium of
designs that goes back to original sources, listing both the source hams,
the source articles, and, where relevant, the source patents for
information.
I have a few notes on the HB9CV at my site as well.
