I had this happen on an Omni VI+, and I found a “bad pin” on a filter. Looked as though some contaminant had been there when the pin was soldered to the filter PCB, and it had not “wetted” well... the pin/solder contact had “fractured”... simply re-wetting it with a soldering iron fixed it.
