[CQ-Contest] Cabrillo Format for RTTY NAQP

Can someone e-mail me privately the required format for this NCJ 
contest.  I used MMTTY barefoot
and it doesn't output a useable log.  If I can create one with an 
editor, I may try it, but it's a hefty log..
Need to know basically what column each of the required fields use.
