[CQ-Contest] Club Affiliation

Hi all.
This year I'm going to be team contesting for the first time in the CQWW 
SSB test. My question is even though I'll be on a team, do I still put my 
club affiliation (Southern Oregon DX Associatiom) on the summary sheet,  
my team name or both?  Thanks and 73
