Rob is exactly right. Tubes have failed even with regular use because they
were never periodically run hot enough.
A well-known case in point was a commercial broadcast transmitter with a
3-500Z driver tube, which never ran red. Those tubes were failing way
before they should have. The fix was to replace it with a 3-400Z, which DID
run red and gettered the gas as it seeped in. Problem solved.
