AD5X has two web articles on QSK conversions using the Nais / Matsushita 
/ Panasonic DS2E-M-DC12V DPDT relay. (The same company seems to use a 
number of different brand names, which surely doesn't help.)

In this article, the relay is used at the amplifier input only, with one 
pole to switch the RF and the other to switch the PA bias. An RJ-1 or 
HC-1 is used to switch the high-power RF output.

The second article is a QSK conversion for the ALS-600. It uses the same 
relay at the output also, with its DPDT contacts paralleled to handle 
the higher current.
