Pete:
I'd recommend you use the car jack once again, but this time wrap a
couple of turns of heavy chain around the rod underneath the vice grips and
loop the chain over the jack's hook. Put wood planking underneath the jack
base to keep it from sinking into the earth and crank the jack. You may
have to move the vice grip and chain lower after lifting the jack every few
inches. This approach worked for me when I had to remove my 18AVT vertical
and ground rod to make way for my tower.

The T-post puller that Joe suggests is a great approach. You'll need to
figure out a choker or some other connection to the hook that would normally
hook onto the T-post. If you set up a choker (a wire or chain with a loop on
one end and a slip knot on the other), make it reach ground level when the
handle is fully up. Because the choker will slide down the ground rod on the
upstrokes, you should be able to walk it out of the ground.
